An exciting opportunity has arisen to work as a Start Well Coordinator within Start Well locality teams.

Over the last 12 months Stockport Family have been strengthening integrated working, the coordination and delivery of early years services through Start Well. 

Start Well brings together early years services to ensure children have the best possible start in life and are ‘school ready’.  
 
The Start Well Coordinators are an integral part of the model, working in locality teams alongside health visitors and early years workers, delivering evidence based interventions in a variety of settings including family homes.  They work closely with communities and key partners to ensure earlier identification of need, timely assessments and coordinated packages of support for children and families.   Start Well teams are linked to health centres, Start Well Hubs and Satellites.  

Key to the role of Start Well Coordinator is supporting the Team Around the Early Years model – a partnership early education and childcare providers, Stockport Family and other agencies who work together to offer support to families with children 0-5 years. 
 
The successful candidate will have significant experience of working in the early years and integrated working.  They must be confident in case holding including complex and challenging cases, completing high quality assessments and developing and reviewing  support plans with families to achieve their goals.  Using restorative approaches, the Start Well Coordinators will have skills in engagement and demonstrate the ability to build positive relationships with families and communities.  

The post is temporary full time (37 hours) until September 2020.
